The clinic’s efforts to accommodate families experiencing economic hardship ran counter to competing pressures from hospital administrators, however. As the economy worsened, the West Clinic became vulnerable within the broader hospital structure because it was not as profitable as other clinics, and Dr.

Although it has since been the target of incisive critiques from social scientists and psychiatrists for disregarding the structural determinants of health,72 this framework has become dominant, even hegemonic, in pain medicine, ensuring a role for multidisciplinary treatment. 73 Where once, in our evolutionary history, pain tended to serve a purpose—to warn of the danger of touching fire, for instance, or to signal an injury that needed attention—chronic pain has lost this biological signaling function.

Focusing on a single case, I trace the causal pathways that clinicians drew between family psychodynamics and a fourteen-year-old patient’s mysterious symptoms. By highlighting the assumptions about family life that influenced the team’s approach to treatment, I show that treatment was aimed, not only at managing pain, but also at prescribing moral and cultural worldviews vis-à-vis adolescent development, family roles, and desirable parenting practices. Finally, chapter 5 considers societal stress as an explanatory frame for pain.
